Personal Protective Equipment (PPE) Resource Request

The North Central Texas Trauma Regional Advisory Council (NCTTRAC ) received a cache of Personal Protective Equipment (PPE) supplies from the Strategic National Stockpile (SNS). This cache is available to organizations and agencies involved in the COVID-19 response in Trauma Service Area (TSA) E based on critical need. The NCTTRAC created new processes to request PPE within TSA E. The NCTTRAC COVID-19 PPE Request Form V5.0 replaces the previous PPE Resources Request form and 213RR v3.0 form and offers new direct entry options.

Hospitals, other health care providers or agency/organizations that experience COVID-19 positive encounters or support vulnerable populations, may submit their request. Read the instructional page thoroughly before submitting.

If you are unable to access the above link, complete the below form and submit it to the Fort Worth Office of Emergency Management.

  1. Email your completed NCTTRAC COVID-19 PPE Request Form V5.0 to the Fort Worth Office of Emergency Management by email at emo99@fortworthtexas.gov
  2. The city's Emergency Management Office will submit a State of Texas Assistance Request (STAR) on your organization's behalf. An email with confirmation of submission and the STAR Request Number will be sent to you.
  3. If resources are available to fulfill your request, NCTTRAC staff will reach out to your organization's main point of contact to discuss the level of fulfillment and to coordinate pickup. NCTTRAC Staff will not contact your organization until they are able to fill your request.

Tornado Shelter Registration

Fort Worth’s Tornado Shelter Registration Program is free and voluntary. It allows residents who own a personal, severe weather shelter in Fort Worth city limits to register their shelter with the city.

Registering your shelter allows the Fort Worth Emergency Operations Center to provide information to first responders during Emergency/Disaster responses. This information gives rescue personnel valuable shelter locations, and is critical should a tornado shelter exit be blocked by debris or structural damage caused by severe weather or other related incidents.

Shelter owners should also include out-of-town accountability contacts as part of your emergency plan. This provides an additional safeguard against being trapped in a shelter.

Always call 911 when someone needs help right away because on an injury or an immediate danger.

You only need to register your shelter once.
