The City of Fort Worth has chosen UNUM to provide group voluntary Long Term Disability (LTD) coverage to eligible employees. To be eligible to enroll, you must be an active employee of the City of Fort Worth and regularly working at least 30 hours per week.
LTD insurance is designed to pay a monthly benefit to you in the event you cannot work because of a covered illness or injury. The city’s plan provides you with several options of income protection to replace 40 or 60 percent of your pre-disability earnings, to a maximum monthly benefit of $6,000 or $9,000, depending on the plan option you choose.
You will also choose a waiting period of either 90 days or 180 days - the amount of time you must wait after being declared disabled in order to collect benefits. The longer the waiting period, the lower the cost of coverage.
Premiums will depend on your current covered annual salary, age and on the percentage of coverage you elect. Premiums will increase as your salary increases and as you age. Premium deductions for LTD are 24 pay periods. (See the example below.)
