Consumer Choice Plan

The Consumer Choice Plan is designed for employees and retirees who like the financial benefit of a Health Savings Account (HSA) and lower monthly premiums.





Health Center Plan

The Health Center Plan provides free and unlimited primary care services to plan members at the three Health Center locations.

Southwestern Health Resources, a collaboration between Texas Health Resources and UT Southwestern Medical Center, is working with the City of Fort Worth to provide an exceptional addition to health care benefits for employees, retirees and their dependents.


Current Employees

Step 1.Same Insurance Provider, Same Health and Benefits Support 

For 2023 the City of Fort Worth will carry the same insurance provider, Meritain Health, an Aetna company.

For personalized health and benefits support, employees and retirees will still contact Accolade. Accolade can help you during this open enrollment period to decide what plan is right for you and your family.  They can help you find a provider and review your medical bills with you.  They will also be the one who pre-certifies things like a surgery that you may have in 2023.

They can also help you coordinate your care when you have a complicated diagnosis that you need help navigating.  In addition, Accolade may reach out to you or members of your family on the city's health plan if you seem to be headed towards a complicated medical procedure.

Step 2.Same Plans with New Perks 

Our 2023 plan includes more than a dozen service providers to meet the healthcare and benefits needs of our employees, retirees and their families.  

New for 2023 - Life insurance up to 8x your annual salary in supplemental life insurance. You can increase by up to 2x your salary or enroll for the first time without submitting evidence of insurability, even if you've been previously declined.

The dental plans are staying the same, right down to the rates as well as long-term disability, short-term disability, life and vision plans, which are staying the same as well. 

FSA enrollment does NOT carry over from year to year. If you have FSA in 2022 and would like to get FSA again next year, log on to cfwbenefits.com to select FSA for 2023. 

City of Fort Worth will still contribute to your HSA $540 for individual and $1000.00 for family. The total HSA contribution limit will increase in 2023 to $3,850 for individual and $7,750 for family. 

Non-Medicare Retirees

No changes for next year, then no action is needed for 2023

Our 2023 plan includes more than a dozen service providers to meet the healthcare and benefits needs of our employees, retirees and their families. 

For 2023 the actual medical plan design will remain the same.

Additionally, for non-Medicare retirees both the dental plan and vision plan rates will be the same. And remember, there will still be a $0 premium option for retirees who select the Consumer Choice Plan option.  

The HSA plan for non-Medicare retirees will remain the same in 2023, $450 for individuals and $1,000 for family.

Medicare Retirees

Same Medicare Advantage Plan for 2023 & Premium Reduction! 

The City of Fort Worth is proud to continue offering the Medicare Advantage plan to all retirees 65 years or older for the 2023 plan year.

Additionally in 2023, the city will continue to offer the Medicare Advantage plan with no monthly premium to retirees hired prior to 1988 or retirees with 25 years or more of service.

The plan offers medical and dental benefits as well as the following:

  • Free membership in Silver Sneakers.
  • $2,000 hearing aid reimbursement once every 36 months. Call 888-432-7464 for assistance.
  • $200 reimbursement for eyewear every 24 months. Call Vision Discount Program at 1-800-793-8616. 

Also, this year the city will continue to offer a vision plan to those on the Medicare Advantage plan through EyeMed. 

If you are making plan changes, you will need to go to cfwbenefits.com to make dental or vision selections. All changes must be made before 11:59 p.m. on Oct. 28, 2022. This includes adding a spouse or dependent to your plan dental or vision plan.

For 2023, there are no changes to the medical plan options offered by the City of Fort Worth, which means employees and non-Medicare (pre-65) retirees will have the same two medical plan options already provided: the Health Center Plan and the Consumer Choice Plan

Since the same plan choices are being offered, that also means that employees and retirees who are making no changes to their current plan elections do not need to take any actions this year. Current elections will automatically be rolled over to the 2024 plan year.

However, if an employee has a flexible spending account (FSA), then they must re-enroll in that part of the health plan every year. It does not roll over.

To verify your doctor, go to Aetna Find-A-Doctor and you can search by entering your ZIP code into the "continue as a guest" section since the new plan coverage does not start until Jan. 1, 2023.
